Procedure

Format the entry using the canonical block:

## YYYY-MM-DD HH:MM — {agent} → {skill}
- input: {input_summary}
- output: {output_summary}
- artifacts: {artifacts as <a class="wikilink wikilink-broken" href="#">wikilinks</a>}
- notes: {notes}

Append to the END of Wiki/Log.md. Never rewrite earlier entries.

Hard rule

Wiki/Log.md is append-only. If a previous entry was wrong, write a follow-up entry that corrects it — don't edit history.
